Limited number of reduced rate sessions based on needs are available, please contact me for details and availability. 
Consultation, Educational, and Speaking Engagements:

Cost varies depending on travel, length of speaking engagement, and other details. Please contact me for details and availability. 

Reduced rates available for nonprofit groups and State/Federal/local agencies (police, schools, etc).
 
Letters, Documents, and Court Related Services:

Records requests are available at a nominal fee of $15.

Letters describing care and/or symptoms written are charged based on the nature of the letter, length, detail, etc. Please consult your counselor to determine the possible benefit, if any, that these letters may provide to your school, attorney, etc. and discuss the cost of writing this letter. 

Court and legal involvement:
CAH and Associates LLC does not current offer expert witnessing or involvement in court proceedings.

Options for Out of Network:
I am happy to provide you with receipts and documentation to submit to your insurance carrier for reimbursement if you choose. Services may be covered in full or in part by your health insurance or employee benefit plan. Please review your coverage carefully, and consult with your insurance provider. Consider the following questions when determining if your insurance will reimburse you for therapy:

•    What are my mental health insurance benefits?
•    Has my deductible been met?
•    How many sessions per year does my health insurance cover?
•    What is the coverage amount per therapy session?
•    Does my insurance require pre-approval from my primary care physician?

Questions regarding not taking insurance and nature of the "superbill" provided for possible reimbursement from insurance company can be asked during the initial meeting or sent via email. 

Cancellations:

Please notify us at least 24 hours in advance if you need to cancel your appointment. If you miss your appointment or fail to notify within 24 hours, there is a $25 fee. It is understood that "life happens" and circumstances are considered at times with every client getting one "freebie" each calendar year. Multiple cancellations or missed appointments will result in discharge from services.
